Direct Supervisor for (9) FTE's (Health Coaches)
  • Assist with daily operations, workflow, advice and suggestions for Health Coaches
  • Weekly/biweekly case reviews for all care management cases open with the health coaches
  • New health coach orientation and training
  • Participate in and lead new staffing Interviews
  • Timecards, schedule/PPL approvals and time keeping management
  • Management of incoming care coordination requests from providers (monitors a pool in Epic and then assigns/distributes work appropriately)
  • Management of Jefferson NJ Hospital discharges daily, assignments to ICM Nurses and Health Coaches to ensure timely follow-up for transition of care calls
  • Management of Jefferson NJ BPCI program from outpatient perspective - collaborate with Melissa/PACC, point person for JNJ related to workflow, process, answering questions, check patient eligibility, manage BPCI case load for nurses in ICM, reporting, analysis
  • Point person for ICM operations and workflows, developing workflows, assisting team members with questions or confusion related to workflows

Manager for Jefferson NJ contracted payers to ICM Team, providing updates, collaboration calls, discuss details, metrics. Manage reporting aspects for Jefferson NJ contracted payers. Manage quality, utilization, and claims data for all payers. Use data to update/adjust workflows to improve processes and Jefferson outcomes (quality, utilization, and cost)

  • Conduct Quality Initiatives related to provider contracts, payer contracts, and inter-departmental metrics
  • Conduct and manage entire ICM Team Case load reviews, identification of high-risk patients for care management
  • Redistributing cases when ICM team members leave the department to ensure continuity of care
  • Participate in Quality Assurance meetings, discussing operations and quality initiatives. Relay to ICM as needed and providing updates on new initiatives
  • Participate in Employee Engagement Council for Jefferson Joint Commission status
  • Completed Jefferson NJ's Mentorship and Development program
  • Biweekly calls with Keith, ICM Project Manager, and Actuarial Informatics for data updates on MSSP, BPCI, etc.
  • Reporting and analyzing Monthly ICM team data and other data as requested (hospital d/c, care coordination, care management, call completion rate, COVID information)
  • Facilitator for Requesting non-JNJ hospital access for new ICM employees
  • Covering for ICM Project Manager on PPL or UPPL
  • Maintain and update extension list for ICM for call transfers
  • Scheduling education sessions/opportunities for non-clinical ICM team members


Bachelors Degree in related field of study.

3-5 years years of direct experience.

Excellent communication skills.

Ability to address needs of culturally and demographically diverse group of patients with chronic disease.

Must be able to work successfully in a team environment.
