Under the direction of the HHC Clinical Manager of the Patient Flow Center the Home Care Liaison collaborates with the host facility case management department to accept referrals. He/she assesses the need for home care or hospice services consistent with admission policies and consistent with CMS regulations. Complies with all policies and procedures of host facility and Jefferson Health Home Care and Hospice (JHHCH) and exhibits professional behavior consistent with the service standards. The Home Care Liaison accepts referrals for patients of all ages.

Qualifications
  • Experience:
  • One + years' medical social work experience in a healthcare related field. Home care or hospice experience preferred.
  • Education & Training:
  • Master's degree in Social Work from a graduate school accredited by the Council on Social Work Education.
  • License, Certification & Registration:
  • Valid driver's license.
  • Other Requirements:
  • Computer skills required, including familiarity with computer software in AJH system. Current auto insurance with proof of coverage required. Must possess a driving record free from major violations within the past five years, and maintain an acceptable driving record with no major violations in accordance with Motor Vehicle Regulations and Abington - Jefferson Health's auto insurance policy. Any and all major violations incurred must be immediately reported to the manager, department head, or Human Resources department.

    Conditions of Employment

    Covid Vaccination is a requirement for employment at Jefferson for employees working at Jefferson's clinical entities or at the University. If you are not currently vaccinated you will be required to receive the vaccination prior to hire date if you are offered employment, unless you request and receive an approved medical or religious exemption from Jefferson.
